ABSTRACT

A Doença de Wilson é uma doença autossômica recessiva que ocasiona no individuo metabolismo deficiente de cobre pelo fígado, compromete as funções hepática e neurológicas, a moléstia evolui de forma progressiva tornando-se potencialmente fatal, caso não seja diagnosticada precocemente e seu tratamento instituído. O presente artigo relata um caso clinico de uma paciente do gênero feminino, melanoderma, 33 anos, diagnosticada com Doença de Wilson-(DW), desde então acompanhada pela equipe multiprofissional durante seu período de hospitalização, em que ocorreu atuação da Odontologia hospitalar nas manifestações clinicas bucais, contribuindo com a adequação do meio bucal através de exames radiográficos e procedimentos realizados no leito. A Odontologia contribuiu com abordagem clínica, intervenção em leito, exames radiográficos, controle de biofilme, possibilitando adequação do meio bucal e melhora do quadro de sangramento gengival. Dessa forma a odontologia se fez presente realizando procedimentos que contribuem na melhora do quadro sistêmico, assim como sendo as "mãos de quem já não possui a autonomia" de realizar atividades básicas como a higiene bucal. Ainda existe um longo caminho de pesquisas e estudos para que o diagnóstico diferencial da DW possa ser realizado de forma mais rápida, visto que se assemelha a outras patologias(AU)


Wilson's disease needs to be better understood, thus reinforcing the need for a precancer diagnosis. Regarding the impairment of the oral cavity due to the evolution of DW, it is well known that the patients affected by this disease receive a dental follow-up, since many are totally dependent on self-care and may cause the individual with DW unsatisfactory oral conditions with repercussion in its systemic picture(AU)

Resumo Introdução A interface implante-pilar protético e a formação dos seus microgaps são aspectos relevantes na transferência das cargas e na resposta biológica, estando ligadas ao sucesso da reabilitação. Objetivo Avaliar microgaps na interface entre a conexão interna do implante do tipo Cone Morse e a superfície do componente protético por meio da microscopia eletrônica de varredura (MEV). Material e método Foram utilizados 20 implantes dentários de tamanho 3,75 × 11,0mm do tipo Cone Morse com seus respectivos pilares protéticos da Singular® (Singular Implants, RN, Brasil). Os munhões retos foram acoplados aos implantes com torque de 32N/cm2 e o conjunto resultante foi emergido em base de Resina Epóxi ES260, para permitir secção longitudinal da amostra. As amostras foram analisadas e os microgaps mensurados no MEV (JEOL JCM-5700, MA, USA), e posteriormente os dados foram analisados. Resultado A média e o desvio padrão dos maiores microgaps foram observados na parte apical do implante nos lados direito e esquerdo, sendo 1,44±2,68 e 1,16±1,49 μm, respectivamente. Os menores microgaps foram na parte superior do implante nos lados direito e esquerdo, sendo 0,60±0,73 e 0,66±0,67 μm, respectivamente. Contudo, no teste de Kruskal-Wallis, não houve diferença estatisticamente significativa entre as regiões dos implantes, tanto para o lado esquerdo (p=0,692) como para o direito (p=0,865). No teste de Mann-Whitney, não houve diferenças estatisticamente significativas entre os lados para as diferentes regiões dos implantes. Conclusão Mesmo com a presença de microgaps na interface implante-pilar protético, estes apresentam tamanho inferior ao que causaria problemas biológicos e mecânicos. As amostras analisadas quanto à sua compatibilidade de encaixe foram satisfatórias.


Abstract Introduction The implant-prosthetic abutment interface and the formation of its microgaps are relevant aspects in load transfer and biological response and are linked to the success of the rehabilitation. Objective To evaluate microgaps at the internal connection interface of the Cone Morse implant and the prosthetic component surface by means of scanning electron microscopy (SEM). Material and method Twenty 3.75 x 11.0 mm Cone Morse dental implants with their respective Singular® prosthetic abutments were used (Singular Implants, RN, Brazil). The straight trunnions were attached to the implants with a 32N/cm2 torque and the ES260 Epoxy Resin-based set emerged to allow longitudinal sectioning of the sample. The samples were analyzed and the microgaps measured in SEM (JEOL JCM-5700, MA, USA), and later the data were analyzed. Result The mean and standard deviation of the largest microgaps were observed in the apical part of the right and left side 1.44±2.68 and 1.16±1.49 μm, respectively. The smallest microgaps were on the upper implant right and left sides 0.60±0.73 and 0.66±0.67 μm, respectively. However, in the Kruskal-Wallis test there was no statistically significant difference between the implant regions, both for the left side (p=0.692) and right side (p=0.865). The Mann-Whitney test, there were no statistically significant differences between the sides for the different regions of the implants. Conclusion Even with the presence of microgaps in the implant-post interface, it presents a smaller size than the one presenting biological and mechanical problems. The samples analyzed for their fitting compatibility were satisfactory.

ABSTRACT: Oral melanoma (OM) is an extremely rare and aggressive malignancy. A 67-year-old patient presented with complains of a slightly symptomatic spot in the mouth since the past 2 years. Extraoral examination revealed left cervical lymphadenopathy, and intraoral examination a blue-black multinodular sessile mass, with irregular margins, involving the attached gingiva of teeth 27 and 28, extending to vestibular sulcus and hard palate, measuring approximately 3.5 cm. The lesion presented focal areas of ulceration. Panoramic radiograph did not show bone involvement. The main diagnostic hypothesis was oral melanoma. Microscopic findings of the incisional biopsy revealed a proliferation of densely pigmented pleomorphic cells, invading the subepithelial connective tissue in sheets or nests showing an organoid pattern. Immunopositivity for S-100, Melan-A and HMB-45 confirmed the diagnosis of melanoma. The patient was referred to an oncology hospital in which multiple metastases were detected, and the patient was subjected to palliative care. Herein we report an OM in advanced clinical stage, and discuss the clinical, morphological and immunohistochemical diagnostic criteria with emphasis on the importance of early diagnosis.


RESUMEN: El melanoma oral (MO) es una malignidad extremadamente rara y agresiva. Un paciente de 67 años acudió a consulta con la queja de una mancha intraoral ligeramente sintomática, presente desde hace dos años. Al examen clínico extraoral, se encontró adenopatía cervical del lado izquierdo, y al examen intraoral, se observó una masa sésil multinodular de color negro azulado, focalmente ulcerada, con bordes irregulares, afectando la encía de los dientes 27 y 28, extendiéndose hasta el surco vestibular y el paladar duro, midiendo aproximadamente 3,5 cm. La radiografía panorámica no mostró involucramiento óseo. La principal hipótesis diagnóstica fue MO. Los hallazgos microscópicos de la biopsia incisional revelaron una proliferación de células pleomórficas densamente pigmentadas, invadiendo difusamente el tejido conectivo en forma de sábanas o nidos con patrón organoide. La positividad inmunohistoquímica para S-100, Melan-A y HMB-45 confirmó el diagnóstico de melanoma. El paciente fue referido a un hospital oncológico, en el cual se le detectaron múltiples metástasis y fue sometido a cuidados paliativos. Este es el reporte de un caso de MO diagnosticado en estado avanzado, en el que se discuten los criterios clínicos, morfológicos e inmunohistoquímicos para su diagnóstico, haciendo énfasis en la importancia del diagnóstico temprano.

ABSTRACT

Introduction : Lip squamous cell carcinoma is a malignant lesion of aggressive behavior, which must be recognized by health professionals to prevent damage to patient's health.Objective:To present incisional biopsy importance as an effective clinical approach for the diagnosis of lip squamous cell carcinoma and actinic cheilitis malignancy as well as the professional's lack of knowledge on these two diseases. Case report:The patient was under medical treatment due to be positive for human immunodeficiency virus and because of the actinic cheilitis on lower lip. Because the patient did not observe the labial lesion regression, he searched for the School of Dentistry of the University Center of the Educational Foundation of Barretos, where he underwent biopsy. This procedure confirmed the diagnosis of lip squamous cell carcinoma. The patient was referred for treatment in a specialized cancer center and has been living with the cancer consequences. Conclusion:The physician and dentist must be aware of the main clinical features of lip squamous cell carcinoma so that they can establish its correct diagnosis and early treatment.

Este estudo avaliou a ocorrência de leveduras do gênero Candida sp além dos principais microrganismos periodontopatogênicos e enterobactérias na saliva, em mucosa e no biofilme aderido à prótese total, correlacionando com aspectos clínicos e condição de higiene bucal de 90 indivíduos edêntulos e portadores de prótese total, por meio de métodos moleculares (PCR). Espécimes clínicos intrabucais foram coletados desses indivíduos após avaliação das condições sócio-econômicas e comportamentais. A microbiota bucal dos pacientes foi caracterizada por meio da obtenção de amostras de biofilme aderido às próteses totais, mucosa e saliva, as quais foram processadas, por meio de PCR. As inter-relações entre os diferentes microrganismos foram determinadas por meio dos testes de Qui-quadrado e Mann- Whitney. Verificaram-se diferenças na ocorrência de Prevotella intermedia e Enterobacteriaceae na saliva dos pacientes edêntulos, o mesmo ocorrendo com Enterobacteriaceae, Camphylobacter rectus e gênero Pseudomonas no biofilme aderido às próteses totais. As condições de higiene bucal e estado de conservação da prótese total precários favoreceram a ocorrência de leveduras do tipo Candida sp, em especial Candida albicans, em níveis estatisticamente significante nas amostras de mucosa e biofilme aderido à prótese total, tornando este dispositivo protético um potencial reservatório de leveduras e bactérias entéricas que podem ser de relevância na patogênese das infecções oportunistas


The aim of this study was to evaluate the occurrence of major periodontopathogenic microorganisms and Enterobacteriaceae and biofilm adhered to the denture, mucosa and saliva in 90 edentulous subjects with complete dentures, using molecular methods (PCR). Clinical specimens were collected from these individuals after assessing the socio-economic circumstances and behavioral. The oral microbiota of patients was characterized by obtaining samples of the biofilm adhered to the dental prothesis and saliva, for detection of major pathogens using PCR. The possibility of inter-relationships between different microorganisms was determined using the chi-square test and Mann-Whitney test. There were differences in the occurrence of Prevotella intermedia and Enterobacteriaceae in the saliva of edentulous patients, likewise, Enterobacteriaceae, Pseudomonas, Camphylobacter rectus and the biofilm attached to denture patients. The conditions for oral hygiene and stat of preservation of prosthesis total precarious favored the occurrence of yeasts of the Candida sp, particularly Candida albicans, statistically significant levels in samples of mucosa and biofilm acceded to total prosthesis, prothetic device, making it a potential reservoir of enteric bacteria and yeasts that may be of relevance in the pathogenesis of opportunistic infections

ABSTRACT

OBJECTIVES: This study evaluated the resistance to antimicrobials of aerobes and facultative anaerobes isolated from patients wearing complete dentures, patients with gingivitis and periodontitis, and periodontally health subjects. MATERIAL AND METHODS: Three hundred and four isolates were tested. The minimal inhibitory concentrations of the drugs were evaluated through the agar dilution method using Mueller-Hinton agar. RESULTS: The most active antimicrobial drugs were the carbapenems (meropenem and imipenem), and resistance to these drugs was restrict to 1.6-2.3 percent of the isolates, as well as ciprofloxacin and rifampin. Microbial resistance to ampicillin, amoxicillin/clavulanic acid, cefoxitin, cephalothin, amikacin, chloramphenicol and nalidixic acid was particularly high. In most cases, the resistance to β-lactams was mediated by the production of hydrolytic enzymes, especially in gram-negative enteric rods, while enterococci did not evidence production of these enzymes. The association amoxicillin/clavulanic acid was not effective in 28.3 percent of the tested isolates. CONCLUSIONS: The results of this investigation confirmed that the oral cavity of patients with periodontitis and gingivitis, and particularly edentulous patients wearing complete dentures could harbor microorganisms with several antimicrobial resistance markers, and these microorganisms are frequently implicated in multiresistant, systemic, oral or nosocomial infections.

ABSTRACT

Introdução: A maioria dos estudos sobre câncer bucal descreve a prevalência dessa doença no gênero masculino e, em geral, naqueles que apresentam hábitos de tabagismo associados ao etilismo. Outra característica é o desenvolvimento dessa patologia, principalmente na 5a e 6a década de vida das pessoas. A borda lateral da língua é o local mais frequente para o aparecimento dessa neoplasia. Pacientes que utilizam próteses dentárias e com carência de higiene bucal também são vítimas do câncer de boca. Objetivo: Esse estudo relata um caso clínico de diagnóstico tardio e o tratamento cirúrgico foi pouco mutilatório, não provocando consequências fonoaudiológicas e mastigatórias para a paciente. Conclusão: O Cirurgião-Dentista deve estar preparado para realização do diagnóstico precoce do câncer de boca para assegurar qualidade de vida aos pacientes.


Introduction: Most of the studies on oral cancer describes the prevalence of this disease in the masculine gender and, in general, in those that present smoking habits associated to the alcoholism. Another characteristic is the development of this disease, mainly in the 5th and 6th decade of people’s life. The lateral border of the tongue is frequently the common most place for this neoplasm appearance. Patient that uses dental prostheses and with no oral hygiene can also be victim of the oral cancer. Objective: This study reports a clinical case of late diagnosis and the surgical treatment which caused little damage; that is, with no harmful consequences on the language and biting abilities of the patient. Conclusion: The Surgeon-Dentist should be aware for the accomplishment of early diagnosis of oral cancer to assure life quality to the patients.

ABSTRACT

Osteomyelitis of the mandible and maxilla are common in developing countries and their treatment may belong-standing and difficult. Thus, the aim of this study was to discuss the main biological aspects of the chronic osteomyelitis of the jaws of especial interest for dental team. These infections are associated with a complex microbiota composed mainly by anaerobic bacteria, sometimes associated with microorganisms originated from the skin and digestive tract. These data suggest that chronic osteomyelitis of the mandible and maxilla should be treated as anaerobic infections in most cases. In addition, local surgical treatments are relevant in the therapy outcome, associated to the use of antimicrobial agents, and the failure to accomplish them is a major cause of treatment failure.


La osteomielitis maxilar y mandibular son comunes en países en desarrollo y su tratamiento puede ser difícil y de larga duración. Así, el objetivo del presente estudio fue discutir los principales aspectos biológicos de especial interés odontológico de la osteomielitis crónica del maxilar. Estas infecciones están asociadas a una microbiota compleja compuesta principalmente de bacterias anaerobias, algunas veces asociadas a microorganismos oriundos de la piel y del tracto digestivo. Estos datos sugieren que la osteomielitis crónica maxilar y mandibular deben ser tratadas como infecciones anaerobias en la mayoría de los casos. Además, tratamientos quirúrgicos locales son relevantes en el éxito de la terapia, asociados al uso de agentes anti-microbianos, e cuyos resultados no satisfactorios representan una de las principales causas de un tratamiento fallo o no adecuado.
